All Butler County agencies are encouraged to submit this form after any request for a medical helicopter response. It is best if this form is filled out by the person designated as landing zone coordinator, or whoever made the helicopter request.
This data collection will help identify common elements between incidents that had communication problems, and also get a better sense of the success/failure rate when it comes to air-ground communication.
